Parents/Guardians may fill out this form to request off-campus time for their student. Please note that students may only go off campus with adult chaperone(s) approved by students’ parents/guardians. Students who are 18 years or older with parent permission may be approved for off-campus time without an adult chaperone, subject to approval by the LSM Student Life Staff. Forms must be submitted 48 hours in advance of the requested off-campus time, to allow the LSM Student Life Staff to prepare and make arrangements.
